The Coast Guard Housing Application and Release from Quarters forms are below, a copy of your PCS orders and CG-4170A are required to be submitted with the Housing Application. Assignment to Government Quarters mandatory, requests for release will be approved when Housing is at 100% occupancy or as needed.
